What Forms Should I Bring with Me?

The last thing you want to do when you arrive for your first appointment is spend time filling out paperwork.

To avoid this, please print and complete both forms below and bring them in with you for your first visit.

I am the Treatment Coordinator at Akron Smile. I will help answer questions about how your dental insurance works and the options for fitting your dental treatment into your lifestyle.

Payment Options

For your convenience, we accept multiple forms of payment. If you have questions regarding your bill, we’re always happy to help.

  • Most major credit cards, including Visa, MasterCard, and Discover
  • Personal checks and cash
  • We accept most major dental insurances. However, we have many patients who choose to visit us, even when we are out of network.
  • At this time, we do not accept Medicaid, Molina or CareSource

Financing Options

Sometimes, patients require an affordable financing plan to pay for their treatment. We are pleased to offer patients the ability to finance through CareCredit. The program offers a customized 12-month interest-free payment plan. Our team is happy to help you navigate the application and approval process.

Whether you’re using insurance, paying cash or financing through Care Credit, payment won’t be an obstacle to your care. We’ll work with you to make whatever arrangement works.

What do I need to bring to my first appointment?

In order to avoid a longer waiting time that involves filling out paperwork, you should fill out your New Patient Forms prior to your visit.  These forms can be found above, and can be brought into the office at the time of your appointment.

If an issue is found during my first exam, will it be taken care of the same day?

Our staff will do everything we can to eliminate the need for a return visit and can take care of most needs in a single appointment.

How long will my first appointment last?

Each appointment can vary slightly in length, but each patient’s first appointment will take approximately 1 hour.

What can I expect during my first dental checkup?

When you come in for a general appointment, we will review your medical and dental history.  Following our review, we will conduct a thorough dental cleaning and a comprehensive exam.

Our team will then share our recommended treatment plan and schedule your next appointment.

What are my options for payment?

At Petry & Walton, we accept most major credit cards, personal checks, cash, and some forms of dental insurance. Learn more about these options here.

If you are planning on using dental insurance, please call your provider to see if we are in their network.
